Community water fluoridation: Untangling facts from fear

Since the long weekend is almost upon us I thought maybe a post today that is quick and easy would be appropriate. The fluoride debate continues to rage and it's sometimes hard to separate fact from fiction.

To help doctors with the current educational situation, the ADA has just posted a great web page that helps doctors to provide necessary information for patients. There are tons of misconceptions out there. People need reliable scientific information. I love technology, but everything has pluses and minuses. The great thing about the Internet is that anyone can have a voice. The bad thing about the Internet is that anyone can have a voice. Because of that it's really hard for curious people to find actually reliable science on a given subject.

Most people count on their dental professionals to trust when it comes to this. Along those lines, the ADA has put together this info for doctors to use. This is yet another reason to be an ADA member. As I frequently say, the profession needs a unified voice that has the ability to talk to lawmakers while also having the resources to provide reliable scientific information.

National Health Care Fraud Takedown Results in 324 Defendants Charged in Connection with Over $14.6 Billion in Alleged Fraud

A short post today, as the real info is from the link. As distressing as it is for me to say this, healthcare fraud exists. Doctors bill for procedures they haven't performed or bill a more expensive procedure than the one that was performed. The problem doesn't always involve doctors. Sometimes it's done by other professionals in the healthcare sector.

The problem erodes trust in people that are supposed to above this sort thing. It's sad, but human nature is human nature and there will always be some bad apples. However, it's good to know that people who do these sort of things don't always get away with it. I happened to come across this story recently and I was stunned at the numbers involved. This is more than twice the number which set the previous record, which was $6 billion.

The Justice Department today announced the results of its 2025 National Health Care Fraud Takedown, which resulted in criminal charges against 324 defendants, including 96 doctors, nurse practitioners, pharmacists, and other licensed medical professionals, in 50 federal districts and 12 State Attorneys General’s Offices across the United States, for their alleged participation in various health care fraud schemes involving over $14.6 billion in intended loss. The Takedown involved federal and state law enforcement agencies across the country and represents an unprecedented effort to combat health care fraud schemes that exploit patients and taxpayers.

Milling is Even Better with the DWX-43W from Roland DGA’s DGSHAPE

As many of you regular readers know, when it comes to digital restorations, I’m a big fan of in-office milling. So that you can properly understand my perspective, let me give you some backstory…

In 2021 I began to look closely at the market for in-office mills. I had actually done in-office milling earlier in my career, but the hardware was very expensive and I wasn’t always happy with the results. However, our practice was growing, patients were getting busier, and technology was making incredible changes in the profession. Zirconia had changed aesthetic restorative treatment and I knew that labs were routinely milling zirconia. I thought “if labs are getting amazing results, I’ll bet we can do the same thing in our office.” With those thoughts in mind, I began exploring my options.

Because of my work in the industry I’ve made a lot of contacts and I began to reach out to them. When I brought up the word “milling”, I kept hearing the same name… Roland’s DGSHAPE. What I didn’t know at the time was that DGSHAPE has a very large footprint in the dental laboratory world. That intrigued me.

In the world of the dental lab, quality is everything. If a lab delivers a crown that doesn’t fit or isn’t aesthetic, the doctor will return it. That simple fact was one of the biggest reasons I decided to put a Roland’s DGSHAPE mill in my office. I knew that if the lab industry was getting well-fitting and aesthetic restorations with DGSHAPE mills, that was the company that I needed to partner with. Security Isn't All About Cyber...

A few years ago I was talking to a nice lady I knew who was a practice management consultant. We were talking about her career and the kinds of problems she helped offices solve. The discussion eventually progressed into discussing overhead and accounts receivable. It was at that point in the conversation when she told me that it wasn't uncommon for a doctor to contact her with cash flow problems of unknown origins.

However, when she dug into the numbers and reports she'd discover... embezzlement. Her feeling was that it happened in 20-25% of the offices she worked with. Now this gal was good with numbers. That was one of her strong points, so maybe that had to do with why a doctor contacted her in the first place, but I was stunned by some of the stories she told me.

It's hard to believe that you can work closely with someone, sometimes for years, only to discover that they were stealing from you. Unfortunately it happens... and it happens a lot. I frequently post about cyber security because I'm a tech guy, but plain old accounting security is just as important.

To bring this into a tech perspective, today's practice management software systems are really strong when it comes to the practice numbers and accounting. It takes a certain amount of effort to cover up the tracks of embezzlement. That's why I was amazed when the consultant told me that the biggest mistake she saw doctors making was simply NOT reviewing the daily reports their software generated. She told me I'd be shocked at how many doctors are given daily reports and simply put them in a file somewhere and never look at them.

She said she had seen that numerous times. She'd be called in because the doctor was struggling to meet payroll and when she asked how often the reports were reviewed she would get nervous or blank looks from the doctor.

So the advice in today's post is "review your day sheet" and the other reports your software generates. I always did because I found the stuff interesting, However a lot of doctors trust someone only to find out that trust was abused. It might have been President Reagan in the 80s, but someone famous once said, "Trust but verify" and I firmly believe in that. A trustworthy person wants you to know they can be trusted.

The whole reason for today's post was an item I happened to see from the Nassau County Police Department in Long Island, NY. It seems that the 34 year old office manager managed to embezzle almost $290,000 from a dental office. The age of the suspect shocked me because it means this wasn't a trickle of dollars over a long period of time. Even if the suspect had started embezzling at the age of 21, that would have been around $22,000 a year... and I doubt the theft had been going on for that long.

So in closing... trust but verify. As you read this, some office somewhere is being embezzled. Do the best you can to not be one of them. Cyber security *is* important, but so is financial security.

While this isn't dental related, it for sure is healthcare related. Veterinary medicine has a lot of similarities to dentistry. DIAGNOcam Vision - Full HD for Incredible Imaging

It's been a bit since I mentioned this device, but I think it bears repeating occasionally for those of you who my be new to the blog or have missed this in the past.

I'm a huge fan of communication and especially communication with patients. This whole concept began in the 90s when my father (who wasn't a dentist) saw an intraoral camera for the first time. He happened to be with me for the demo of a cutting edge intraoral imaging system. Back then it was standalone and on a cart. Heck, back then there was no such thing as computers in the operatory so the entire imaging system was wheeled around on a cart.

My dad was amazed at what he could see on the screen. At the time, a simple camera system was $15.5K and the moment the sales guy left my dad turned to me and said, "When are you going to buy it? You have to get one!!!"

This was early in my career and I didn't have the money to buy it. My dad offered to loan me the money and told me that if I didn't buy it immediately, I was making a mistake. He then went on to explain to me that if I could show people their problems and helped them understand the reason for treatment, I would be able to help more people. He explained that it's all about communication. My dad was in sales and he understood that inherently. The next week, I had a camera in my office and I was shocked at just how right my dad had been.

In a nutshell, better informed patients make better decisions about their care. It's just that simple. For that reason, I've always loved any kind of tech that allows me to better communicate with my patients.

Which brings us to today's post. Not too long ago, Kavo brought a new device to market. It's called the DIAGNOcam Vision Full HD and it is a terrific piece of technology. For the first time in the history of the profession a doctor or team member can pick up a single device and acquire every image possible that doesn't require x-rays. The DIAGNOcam Vision functions as an intraoral camera, a transilluminator, and a visual caries detection device.

The unit not only takes all three types of images, but it can be configured to take all three types with the push of a single button. Basically you put the device on top of the tooth, push a button and pop, pop, pop... all three images appear.

Camera

The camera, as the name implies, is a true HD image. The illumination for photos is bright and images have terrific contrast. The camera also has true autofocus so the images are always sharp and clear. The autofocus works so well, that it can even be used as an extra oral camera for patient photos. The best part is that because of the autofocus, there is no distortion or 'fish-eye' even on images taken of the patient's face.

Transillumination

Transillumination is a great diagnostic tool. It allows the operator to see small areas of interproximal decay as well as cracks and fracture lines. There are a lot of devices available to perform that function. However, others are like tiny flashlights that let the operator shine the light from the buccal or the lingual with no way of actually taking a photo of the situation. The DIAGNOcam Vision handles it in a much more high tech way. The device has small flexible arms (see the photo at the top of this post) that gently grip the tooth. Those arms have fiberoptics in them and the device uses an invisible NIR (near infrared) wavelength that penetrates the tooth. While invisible to the human eye, it can be seen by the camera in the device. This lets the device capture an image from the occlusal surface while the tooth is illuminated from the buccal and lingual with the NIR.

The images are crisp and highly diagnostic. The NIR provides incredible illumination with low intensity which means there are no overly bright spots. This is a great way to monitor small areas, especially when the doctor is using remineralization therapy. It allows for monitoring of an area on a regular basis since no x-rays are required. It's also incredibly useful for pediatric patients or patients that are pregnant.

Caries Fluorescence

Finding caries when it is tiny is another way to conservatively manage patients. Under certain wavelengths of light, metabolic byproducts, called porphyrins, fluoresce. The DIAGNOcam Vision uses one of those wavelengths to bathe the tooth in a purple bluish light that lets the operator actually see where those bacteria are located. After polishing the teeth to remove plaque, the light can indicate where live bacteria are causing breakdown of tooth structure. This means caries can be located when the area is tiny. If a restoration is required, this means more conservation of healthy tooth structure. It can also help with remineralization therapy because areas can be frequently monitored. The images can be saved and compared to create a virtual time lapse of progression or remineralization.

Wrapping Up

I was lucky enough to be able to clinically evaluate the DIAGNOcam Vision before it was publicly available and I was impressed. The device is easy to use, well designed, and is a *great* communication tool. Being able to collect diagnostic images quickly and easily while then allowing the patient to see their situation is a remarkable win/win for the practice. My dad was right. Better informed patients make better decisions. Combining that fact with the ability of the practioner to also be better informed through all of these imaging modalities leads to better outcomes.

An Unusual Side Effect - Ozempic Tongue

Many medications can affect the oral cavity. They can decrease salivary flow, cause more calculus to form, and a myriad of other things. Now with so many patients taking glucagon-like peptide-1 receptor agonists a new thing has occurred in some patients.

Some folks are reporting a situation that is being termed Ozempic Tongue. The symptoms are a a persistent bitter or metallic taste in their mouths. Obviously, anything that absorbs into the body has the potential to then be distributed throughout. Experts think it stems from the medication being absorbed into the body and then getting into the salivary glands.

It doesn't seem to be causing any problems in the mouth other than a persistent bad taste and it's not something that every patient deals with. Still it's a great idea for doctors to be aware of it. It's also good to discuss this potential side effect with patients that are taking these medications.

Simplify Shade Matching with Rite-Lite Pro™

One of the most difficult things to do in dentistry is to shade match a single shade in the cosmetic zone. However, matching two teeth isn't really that much easier. The proper shade match is critical and it's never easy. If you've been practicing dentistry for any amount of time and run into this problem, you know exactly what I'm talking about.

Anterior crowns are especially difficult. Our aesthetic materials have made this a bit easier in the last 10-15 years, but even with all of the advancements, it is still a challenge. Simply put, the design of human tooth consists of enamel over dentin. Enamel is translucent and dentin is not. That means that light striking the tooth is reflected in ways that are hard to mimic with manmade materials. Things such as hue, value, and chroma all come into play... and they can present some challenges.

Part of the challenge is the light the shade match is done under. A smile isn't just seen in sunlight, it's also seen in a variety of manmade wavelengths and those are probably the biggest reason for the difficulty in shade matching. A crown can be perfect under the lights of a dental office, but when seen under the lights in a home, an office environment, or restaurant, that shade can be affected by the ambient light. Many doctors will match the shade and view the patient under a variety of lighting environments to try and maximize the aesthetics. Personally I've taken shades in the operatory, in the reception area, and the parking lot on difficult shade cases.

How can a doctor view the smile under a variety of shade environments and decrease the chances of a crown not matching? One of the easiest ways I've found to do this is with the Rite-Lite PRO™ from AdDent.

There is a measure of how accurately a light source reveals the colors of an object compared to natural sunlight. That measure is referred to as CRI (Color Rendering Index) and can be incredibly helpful in these types of situations.

The Rite-Lite PRO is a shade matching device that uses a variety of high CRI LEDs at different specific wavelengths that allow the tooth to be visualized correctly. A variety of different wavelengths can help increase the accuracy of the shade match. The LEDs provide 3 modes of shade taking.

Color-corrected light at a color temperature of 5500° Kelvin. This is considered 'true north' light.
Incandescent-room light at 3200° Kelvin that is found in many indoor environments.
Ambient list at 3900° Kelvin. This is a combination of room light and daylight.

There is also a black light setting for matching restorations to natural dentition fluorescence.

As you can see from the image at the top of this post, the Rite-Lite PRO looks like a large magnifying glass. The LEDs are setup in a ring with a large viewing port in the middle of the ring. This lets the doctor or shade taker see the teeth evenly illuminated with each specific wavelength. You can also purchase a polarizing filter to remove any surface reflection. Pressing a button on the handle cycles the LEDs through the different wavelengths.

The ring illuminator is attached to a handle that contains a rechargeable Li-ion battery that provides constant power output which means the light is always at its correct brightness and intensity. This allows the shade taker to know everything is the same... every time.

If you've ever struggled to get the shade match you and your patient expect, I highly recommend this device. It makes shade taking *much* easier. As I've said before, the smart people at AdDent make some of the best "better mousetrap" devices I've seen. This is one of them!

This one has been a true labor of love and we've worked hard behind the scenes to put this together. The first episode is with Sean Ryan, the CEO of Medidenta and Medidenta Digital. They are the folks who are sponsoring the podcast so I thought he would be a great first guest.

The first episode gives you an idea of why we partnered together to do this. I thought, who better to explain about the emphasis on education, than they guy who runs the company that sponsors it? Navigating Rough Waters: iCoreConnect Files for Chapter 11

In recent news that has caught the attention of the healthcare technology sector, iCoreConnect Inc., a prominent provider of cloud-based software solutions for healthcare, including many dental practices, filed for Chapter 11 bankruptcy protection. The voluntary petitions were filed by iCoreConnect Inc. and its wholly-owned subsidiary, iCore Midco Inc., in the United States Bankruptcy Court for the Middle District of Florida on June 2, 2025.

This move comes as the company aims to "retain control over their assets and operations, optimize their organizational structure, restructure their balance sheets, and strategically position themselves for long-term success,"1 as stated in their SEC filings. The filing reveals that iCoreConnect had over $10 million in outstanding note payables that were in default. The company also faced challenges including a weak financial health score and a concerning current ratio, as highlighted by financial analysts.

For many dental practices, iCoreConnect's suite of products – encompassing claims management, eligibility verification, practice analytics, secure email, and IT services – has become integral to their daily operations. The news of the company's Chapter 11 filing naturally raises questions and may cause concern among its users. However, Chapter 11 is a reorganization bankruptcy, not a liquidation. This means iCoreConnect intends to continue its operations while it works to restructure its debt and financial obligations.

The company has already requested that the Bankruptcy Court jointly administer the cases to enhance procedural efficiency and plans to submit additional "first-day motions" to ensure business continuity. These motions typically seek court approval for essential actions like paying employee wages, managing utilities, and using cash collateral to keep the business running during the reorganization process.

While the reasons for the filing appear to be financial restructuring and optimizing its internal balance sheet, iCoreConnect's commitment to its cloud-based software solutions for dental practices remains. For current users, the immediate impact should ideally be minimal as the company navigates this legal process with the stated goal of emerging stronger. However, it’s always prudent for practices relying on critical third-party software to stay informed about their vendors' stability and any communications regarding service continuity.

This development underscores the dynamic nature of the technology landscape, even for established providers. For iCoreConnect, it represents a strategic effort to recalibrate its financial footing and secure its future in the competitive healthcare software market.

ICD Global Oral Health Leadership Institute Holds Stakeholders’ Day Program

The International College of Dentists (ICD) hosted its second Stakeholders’ Day program in Boston, Massachusetts, USA, on April. 29, 2025. The program and celebratory dinner were held on the campus of Harvard School of Dental Medicine (HSDM), and were made possible by a donation from industry partner Henry Schein. The Stakeholder’s Day was a key event for ICD’s Global Oral Health Leadership (GOHL) Institute, that was launched in December 2024 in partnership with Henry Schein. GOHL is designed to support global leadership in oral health and to encourage meaningful discussions that foster collaboration and sustainable public-private solutions.

Stakeholders Day provided an opportunity for the 2025 GOHL delegates to engage with thought leaders, innovators and activists in oral health and other sectors, as well as promote the GOHL Institute objectives to foster a global community of practice, lead system change through evidence and innovation, empower transformational leadership, bridge sectors, and catalyze cross-sectoral impact, to interested collaborators. The event kicked off with two panel discussions led by Dr. Marko Vujicic, Chief Economist and Vice President of the Health Policy Institute, American Dental Association, and Dr. Greg Chadwick, President of the FDI World Dental Federation and Dean of the East Carolina University School of Dental Medicine, where they addressed evidence-based advocacy and policy in the face of challenging contexts, interdisciplinary collaboration to achieve system-level change, and actionable steps through innovation and sustainability.

“I feel we’re at the beginning of a ‘Golden Era’ of oral health internationally. A decade ago, we did not have compelling evidence that investing in oral health, for example, saves money on pregnancy outcomes or reduces healthcare costs among people with diabetes. There is a very strong value proposition for oral health today that we didn’t have a decade ago. There’s significant momentum, and a lot of it has to do with economics. The fact that we can show there is a financial ROI to investing in oral health is really important in policy circles,” Dr. Vujicic said during his keynote. “This ‘Golden Era’ opportunity for oral health is inconsistent with the status quo financing and delivery models that exist. So that is the tension I feel the leaders of the profession need to address, a fundamental realignment in what oral health care focuses on.”

The program culminated with a dinner, enabling dialogue between changemakers in oral health. Keynote speaker Dr. Hugh Silk, family physician in the Dept. of Family Medicine and Community Health at the University of Massachusetts Chan Medical School, gave a powerful presentation about leading a pathway to change through curiosity, creativity and love, and his journey of incorporating oral health into overall health to reach more patients where they are at.

The Dean of the Harvard School of Dental Medicine, Dr. William Giannobile, also addressed guests on the pride of being part of an important collaborative initiative like this.

“As we all know, right now is such a challenging time in academia, in education and in research, so to see a program dedicated to global oral leadership is more important now than ever,” said Dr. Giannobile. “Part of the vision of the HSDM is to transform access to healthcare and advance science to prevent and find cures for oral diseases and conditions, and part of our mission is really to foster that community of diverse global leaders in research, education and clinical innovation to really integrate medicine and oral health.”

Stakeholders’ Day showcased the important interdisciplinary approach of GOHL, while challenging prevailing norms and inspiring actionable change in global oral health. Distinguished guests from across oral health participated, including the 2025 GOHL delegates representing 10 countries, ICD Fellows, and prominent industry and academic figures.

I am all in on 3D printing!!! In January 2024, I was approached by a couple of companies who wanted to get my opinion on their 3D printing system. I had worked with the tech before, but I was interested to see how it had evolved. As I began to tinker and experiment with those systems, I was blown away at what was possible.

Ever since then, I've worked on other projects, but I've always come back to 3D printing. Why, you may ask? Because what you can do is flat out amazing. I remember when I first saw 3D printing at a dental meeting. It was at the ADA meeting in Washington, DC. That would have been probably 2016 or 2017. It was amazing even then, but things have changed so dramatically since then. Basically, if you haven't seem 3D printing for dentistry lately, you're missing a lot.

When CBCT first started to go mainstream in the profession around 2008 we saw the very beginning of what 3D could do. Now with the amazing progress we've had with intraoral scanning and AI, we're perched on the edge of dentistry's next big thing. We're going to see 3D patient info merge into all kinds of systems that will work together to give us better patient outcomes than we ever thought possible.

Up until recently the problem with 3D printing was the design, but AI has made incredible inroads into that aspect of things. Designing is now easy!!!

Along those lines, I've partnered with the smart folks at Medidenta to do some hands-on programs at their state of the art facility in Las Vegas. One of those programs is being held on June 21-22nd. The ADA Bids Farewell to their Annual Meeting - 2025 Smilecon Will be the Last

Last week I had a post that discussed the recent CDA Anaheim meeting. As I stated there, in-person industry trades hows have suffered a lot since the pandemic of 2020. However, some meetings seem to be recovering... at least a bit.

Attendance at many major meetings has dropped significantly since Covid. However, it wasn't just social distancing that caused the slowdown. Our world is showing amazing progress in technologies and the ways we communicate and in-person meetings are one of the victims. Since the pandemic, overall trade show attnedance has been dropping. Now comes word that on Thursday May 23, 2025 the American Dental Association announced that the 2025 Smilecon meeting will be the last one.

The first video ever played on MTV on August 1, 1981 was "Video Killed the Radio Star" by The Buggles, and that was prophetic. Now online education has just killed what was once the premier in-person trade show in dentistry. Young doctors no longer feel the need to close their offices for several days and travel to far away places. YouTube, TikTok, and Instagram feed the desire for learning and CE credits can be earned through viewing webinars.

Last week I mentioned that the CDA seems to be on the rebound. I still believe some of the larger in-person meetings will continue. I think there is something to be said for a meeting at basically the same time and at the same location year after year. When you are planning on attending a meeting, being able to know the location and not having to travel can be big reason to go. By trying to move every year, Smilecon wanted to make itself available to the majority of their members who are spread throughout the country. The goal was that no matter where you lived, every few years the meeting was at least in your general vicinity. That was an admirable idea, but I think that might have lead to it's demise. However, this has been something many in the industry have predicted for a few years. Recent attendance has been down quite a bit both in attendance from doctors and booths manned by exhibitors. The last Smilecon that I attended was in Orlando in 2023 and everyone noticed that the attendance was light. And just to be clear, this is not an indictment of Smilecon. Many meetings have suffered from low attendance since 2020 and I feel we're going to see more contraction in the future.

However, in their press release the ADA mentioned:

The Governance and Organizational Manual of the ADA requires that the ADA hold an annual scientific session. The ADA is exploring new meeting possibilities and programs to advance oral and overall health by keeping dental professionals up to date on the latest scientific guidelines and technical innovations. Further information will be shared when it is available.

That means that the association will still continue to explore options for some type of meeting, which in my opinion is good news. Education is critical to the profession, and the ADA has frequently helped provide some of the best education there is in dentistry. I'm hopeful that whatever comes next in the required scientific session is successful. It will probably be smaller and not such an expensive line item in the budget, but that's ok by me.

More importantly, dentistry needs the united voice that is the ADA. There is a huge benefit to the profession speaking with a single voice, and that voice is the ADA. Dentistry has benefitted greatly from the hard work of the ADA. Now they can take the financial resources that were being poured into the meeting and use it for other projects. I'm hopeful of what they can with that cash.

Here's a bit of nostalgia... in 1998 what was then called "The ADA Annual Session" was flying high. This was in the very early days of the Internet and computers in healthcare. Back then I had started taking computers apart in my office and had begun writing for a newsletter (photocopied and delivered via USPS) called 'Tools R Us' and published by a good friend Dr. Bob Davis.

Back then my writing for Bob's newsletter on technology in dentistry had somehow come to the attention of some folks at the ADA headquarters in Chicago. With technology poised to have a major impact on the healthcare industry, the ADA had decided to have a special day on the subject as part of their Annual Session. The first ever ADA Technology Day was in San Francisco in 1998.

I vividly remember my first lecture for the ADA. One of the things I spoke about was a new technology that allowed users to connect peripheral devices to their computers. It was called "Universal Serial Bus" and was abbreviated USB. I clearly remember talking about it while audience members furiously scribbled in their notebooks. There were some vendors displaying leading edge tech and the one thing I was really blown away by was a 17 inch flat screen display being shown by Intel. I had seen pictures of them, but this was my first time to actually see one in person. The retail price was $7000!

I was really lucky. At that meeting I made a lot of good industry contacts, which is important for anyone who wants to educate their peers. I spoke at every ADA meeting for about the next 25 years. Often getting into the show meant going through the doors in a mass of humanity. It was incredible.

As someone who has made a career out of being in love with tech I can tell you without a doubt, everything evolves and as progress is made older ideas and tools are replaced. The evolution of broadband connections and video has changed the way many people receive their information. I still think some in-person meetings will survive, but we've now entered a time when we are going to see contraction in their numbers. The ADA Annual Session was a real boon to my career as Technology Evangelist and I'm sad to see it go. However it makes no sense to spend resources on a meeting that few attend. At the end of the day, this was a business decision. I get that and I support the decision, but I can still be nostalgic for those good old days.

Artificial intelligence continues to make deeper and deeper inroads into dentistry. More and more offices are embracing this change to help with diagnosis. As we do, the entire process of how the profession analyzes radiographs is changing.

I worked alone in a solo practice for about 18 years before bringing in another doctor. Looking back, I wish I would have made that decision much earlier. One of the immediate benefits was being able to look a an x-ray and ask another set of trained eyes the question "do you see any pathology on this image?" Having another set of eyes on target is a remarkable benefit.

I started working with some dental AI startups back in 2015 (I can't believe it was *ten* years ago) and I immediately saw the benefit. Even though the interface back then was clunky and required a lot of work on my end to get the images loaded into the AI platform, I was hooked. AI doesn't diagnose, it gives the doctor a gentle nudge to say "did you look really closely here?" Because of those gentle nudges, doctors are making better decisions.

Now the UCLA School of Dentistry is bringing Pearl into their clinic. That means that dental students are going to get exposure to how to practice as the profession evolves. I'm proud and excited by this decision. Young doctors are often flooded by new things as soon as the graduate. In my opinion, our dental schools turn out terrific clinicians, but those students only get limited time with 'state of the art' technologies simply because of the time involved to learn them and the costs to the schools to provide them. Ivoclar Announces OptraGate 2

Today's post is short, but that's because the majority of what you need to know will be in the link at the bottom.

Anyone who performs dentistry in the anterior knows that retraction is critically important. Over the years, there have been several products released that can help solve that problem, but the one that everyone knows is Ivoclar's OptraGate. It was the first thin and flexible lip retraction device that I ever worked with... and I loved it. Now the company has announced the new and improved OptraGate 2.

The Optragate 2 is a non-latex lip and cheek retractor that, as you can see in the image above, provides a clear view for treatment in the anterior segment of the mouth. There are flexible rings in the material that allow the device to be incredibly flexible. Basically it is folded almost in half, then the interior ring is placed under the lip and as the device unfolds, it expands and retracts the lips.

The biggest difference in the new design is the addition of a third flexible ring located midway between the inner and outer rings. Ivoclar says the addition of this third ring "reduces compressive strength on the jawbone".

From that description, I'm guessing that what they mean is that it helps keep the inner ring from pressing against the facial aspect of the alveolar ridge as it holds the lip back. I haven't had a chance to to try the product yet, so that is speculation. However, as a patient I did have the original OptraGate used on me by a friend and I did notice some tenderness along the labial plate due to the pressure of the internal ring. To be clear, it was not bad and certainly hasn't ever kept me from using one when needed. I applaud Ivoclar for continuing to innovate on a product that is a simple design. Everything can be improved and it shows how the company is always looking to make things better.
