As distressing as it is for me to say this, healthcare fraud exists.&nbsp; Doctors bill for procedures they haven't performed or bill a more expensive procedure than the one that was performed.&nbsp; &nbsp;The problem doesn't always involve doctors.&nbsp; Sometimes it's done by other professionals in the healthcare sector.</p><p>The problem erodes trust in people that are supposed to above this sort thing.&nbsp; It's sad, but human nature is human nature and there will always be some bad apples.&nbsp; However, it's good to know that people who do these sort of things don't always get away with it.&nbsp; I happened to come across this story recently and I was stunned at the numbers involved.&nbsp; We were talking about her career and the kinds of problems she helped offices solve.&nbsp; The discussion eventually progressed into discussing overhead and accounts receivable.&nbsp; It was at that point in the conversation when she told me that it wasn't uncommon for a doctor to contact her with cash flow problems of unknown origins.</p><p>However, when she dug into the numbers and reports she'd discover... embezzlement.&nbsp; Her feeling was that it happened in 20-25% of the offices she worked with.&nbsp; Now this gal was good with numbers.&nbsp; That was one of her strong points, so maybe that had to do with why a doctor contacted her in the first place, but I was stunned by some of the stories she told me.</p><p>It's hard to believe that you can work closely with someone, sometimes for years, only to discover that they were stealing from you.&nbsp; Unfortunately it happens... and it happens a lot.&nbsp; I frequently post about cyber security because I'm a tech guy, but plain old accounting security is just as important.&nbsp;&nbsp;</p><p>To bring this into a tech perspective, today's practice management software systems are <i style="font-weight: bold;">really strong </i>when it comes to the practice numbers and accounting.&nbsp; It takes a certain amount of effort to cover up the tracks of embezzlement.&nbsp; That's why I was amazed when the consultant told me that the biggest mistake she saw doctors making was simply NOT reviewing the daily reports their software generated.&nbsp; She told me I'd be shocked at how many doctors are given daily reports and simply put them in a file somewhere and <b><i>never look at them.</i></b></p><p>She said she had seen that numerous times.&nbsp; She'd be called in because the doctor was struggling to meet payroll and when she asked how often the reports were reviewed she would get nervous or blank looks from the doctor.</p><p>So the advice in today's post is "review your day sheet" and the other reports your software generates.&nbsp; I always did because I found the stuff interesting, However a lot of doctors trust someone only to find out that trust was abused.&nbsp; It might have been President Reagan in the 80s, but someone famous once said, "Trust but verify" and I firmly believe in that.&nbsp; A trustworthy person wants you to know they can be trusted.</p><p>The whole reason for today's post was <a href="https://www.pdcn.org/CivicAlerts.aspx?AID=14184" target="_blank">an item I happened to see from the Nassau County Police Department in Long Island, NY</a>.&nbsp; It seems that the 34 year old office manager managed to embezzle almost $290,000 from a dental office.&nbsp; The age of the suspect shocked me because it means this wasn't a trickle of dollars over a long period of time.&nbsp; Even if the suspect had started embezzling at the age of 21, that would have been around $22,000 a year... and I doubt the theft had been going on for that long.</p><p>So in closing... trust but verify.&nbsp; blog or have missed this in the past.</p><p>I'm a huge fan of communication and especially communication with patients.&nbsp; This whole concept began in the 90s when my father (who wasn't a dentist) saw an intraoral camera for the first time.&nbsp; He happened to be with me for the demo of a cutting edge intraoral imaging system.&nbsp; Back then it was standalone and on a cart.&nbsp; Heck, back then there was no such thing as computers in the operatory so the entire imaging system was wheeled around on a cart.</p><p>My dad was amazed at what he could see on the screen.&nbsp; At the time, a simple camera system was $15.5K and the moment the sales guy left my dad turned to me and said, "When are you going to buy it?&nbsp; <b>Y<i>ou have to get one!!!"</i></b></p><p>This was early in my career and I didn't have the money to buy it.&nbsp; My dad offered to loan me the money and told me that if I didn't buy it immediately, I was making a mistake.&nbsp; He then went on to explain to me that if I could show people their problems and helped them understand the reason for treatment, I would be able to help more people.&nbsp; He explained that it's all about communication.&nbsp; My dad was in sales and he understood that inherently.&nbsp; The next week, I had a camera in my office and I was shocked at just how right my dad had been.</p><p>In a nutshell, better informed patients make better decisions about their care.&nbsp; It's just that simple.&nbsp; For that reason, I've always loved any kind of tech that allows me to better communicate with my patients.</p><p>Which brings us to today's post.&nbsp; Not too long ago, Kavo brought a new device to market.&nbsp; It's called the DIAGNOcam Vision Full HD and it is a terrific piece of technology.&nbsp; For the first time in the history of the profession a doctor or team member can pick up a single device and acquire every image possible that doesn't require x-rays.&nbsp; The DIAGNOcam Vision functions as an intraoral camera, a transilluminator, and a visual caries detection device.&nbsp;&nbsp;</p><p>The unit not only takes all three types of images, but it can be configured to take all three types with the push of a <i style="font-weight: bold;">single button.&nbsp; </i>Basically you put the device on top of the tooth, push a button and pop, pop, pop... all three images appear.</p><p><br /></p><p><b>Camera</b></p><p>The camera, as the name implies, is a true HD image.&nbsp; The illumination for photos is bright and images have terrific contrast.&nbsp; The camera also has true autofocus so the images are always sharp and clear.&nbsp; The autofocus works so well, that it can even be used as an extra oral camera for patient photos.&nbsp; The best part is that because of the autofocus, there is no distortion or 'fish-eye' even on images taken of the patient's face.</p><p><b>Transillumination</b></p><p>Transillumination is a great diagnostic tool.&nbsp; It allows the operator to see small areas of interproximal decay as well as cracks and fracture lines.&nbsp; There are a lot of devices available to perform that function.&nbsp; However, others are like tiny flashlights that let the operator shine the light from the buccal or the lingual with no way of actually taking a photo of the situation.&nbsp; The DIAGNOcam Vision handles it in a much more high tech way.&nbsp; The device has small flexible arms (see the photo at the top of this post) that gently grip the tooth.&nbsp; Those arms have fiberoptics in them and the device uses an invisible NIR (near infrared) wavelength that penetrates the tooth.&nbsp; While invisible to the human eye, it can be seen by the camera in the device.&nbsp; This lets the device capture an image from the occlusal surface while the tooth is illuminated from the buccal and lingual with the NIR.</p><p>The images are crisp and highly diagnostic.&nbsp; The NIR provides incredible illumination with low intensity which means there are no overly bright spots.&nbsp; This is a great way to monitor small areas, especially when the doctor is using remineralization therapy.&nbsp; It allows for monitoring of an area on a regular basis since no x-rays are required.&nbsp; It's also incredibly useful for pediatric patients or patients that are pregnant.</p><p><b>Caries Fluorescence</b></p><p>Finding caries when it is tiny is another way to conservatively manage patients.&nbsp; Under certain wavelengths of light, metabolic byproducts, called porphyrins, fluoresce.&nbsp; The DIAGNOcam Vision uses one of those wavelengths to bathe the tooth in a purple bluish light that lets the operator actually see where those bacteria are located.&nbsp; After polishing the teeth to remove plaque, the light can indicate where live bacteria are causing breakdown of tooth structure.&nbsp; This means caries can be located when the area is tiny.&nbsp; If a restoration is required, this means more conservation of healthy tooth structure.&nbsp; It can also help with remineralization therapy because areas can be frequently monitored.&nbsp; The images can be saved and compared to create a virtual time lapse of progression or remineralization.</p><p><b>Wrapping Up</b></p><p>I was lucky enough to be able to clinically evaluate the DIAGNOcam Vision before it was publicly available and I was impressed.&nbsp; The device is easy to use, well designed, and is a *great* communication tool.&nbsp; Being able to collect diagnostic images quickly and easily while then allowing the patient to see their situation is a remarkable win/win for the practice.&nbsp; However, matching two teeth isn't really that much easier.&nbsp; The proper shade match is critical and it's never easy.&nbsp; If you've been practicing dentistry for any amount of time and run into this problem, you know exactly what I'm talking about.</p><p>Anterior crowns are especially difficult.&nbsp; Our aesthetic materials have made this a bit easier in the last 10-15 years, but even with all of the advancements, it is still a challenge.&nbsp; Simply put, the design of human tooth consists of enamel over dentin.&nbsp; Enamel is translucent and dentin is not.&nbsp; That means that light striking the tooth is reflected in ways that are hard to mimic with manmade materials.&nbsp; Things such as hue, value, and chroma all come into play... and they can present some challenges.&nbsp;</p><p>Part of the challenge is the light the shade match is done under.&nbsp; A smile isn't just seen in sunlight, it's also seen in a variety of manmade wavelengths and those are probably the biggest reason for the difficulty in shade matching.&nbsp; A crown can be perfect under the lights of a dental office, but when seen under the lights in a home, an office environment, or restaurant, that shade can be affected by the ambient light.&nbsp; Many doctors will match the shade and view the patient under a variety of lighting environments to try and maximize the aesthetics.&nbsp; Personally I've taken shades in the operatory, in the reception area, and the parking lot on difficult shade cases.</p><p>How can a doctor view the smile under a variety of shade environments and decrease the chances of a crown not matching?&nbsp; One of the easiest ways I've found to do this is with the <a href="https://addent.com/rite-lite-pro/" target="_blank">Rite-Lite PRO™</a> from <a href="http://AdDent.com">AdDent.</a></p><p>There is a measure of how accurately a light source reveals the colors of an object compared to natural sunlight.&nbsp; The LEDs are setup in a ring with a large viewing port in the middle of the ring.&nbsp; This lets the doctor or shade taker see the teeth evenly illuminated with each specific wavelength.&nbsp; You can also purchase a polarizing filter to remove any surface reflection.&nbsp; Pressing a button on the handle cycles the LEDs through the different wavelengths.</div><div><br /></div><div>The ring illuminator is attached to a handle that contains a rechargeable Li-ion battery that provides constant power output which means the light is always at its correct brightness and intensity.&nbsp; This allows the shade taker to know everything is the same... every time.</div><div><br /></div><div>If you've ever struggled to get the shade match you and your patient expect, I highly recommend this device.&nbsp; The voluntary petitions were filed by iCoreConnect Inc. and its wholly-owned subsidiary, iCore Midco Inc., in the United States Bankruptcy Court for the Middle District of Florida on June 2, 2025.</p><p>This move comes as the company aims to "retain control over their assets and operations, optimize their organizational structure, restructure their balance sheets, and strategically position themselves for long-term success,"1 as stated in their SEC filings. The filing reveals that iCoreConnect had over $10 million in outstanding note payables that were in default. The company also faced challenges including a weak financial health score and a concerning current ratio, as highlighted by financial analysts.</p><p>For many dental practices, iCoreConnect's suite of products – encompassing claims management, eligibility verification, practice analytics, secure email, and IT services – has become integral to their daily operations. The news of the company's Chapter 11 filing naturally raises questions and may cause concern among its users. However, Chapter 11 is a reorganization bankruptcy, not a liquidation. This means iCoreConnect intends to continue its operations while it works to restructure its debt and financial obligations.</p><p>The company has already requested that the Bankruptcy Court jointly administer the cases to enhance procedural efficiency and plans to submit additional "first-day motions" to ensure business continuity. The program and celebratory dinner were held on the campus of Harvard School of Dental Medicine (HSDM), and were made possible by a donation from industry partner Henry Schein. The Stakeholder’s Day was a key event for ICD’s Global Oral Health Leadership (GOHL) Institute, that was launched in December 2024 in partnership with Henry Schein. GOHL is designed to support global leadership in oral health and to encourage meaningful discussions that foster collaboration and sustainable public-private solutions.&nbsp;</p><p>Stakeholders Day provided an opportunity for the 2025 GOHL delegates to engage with thought leaders, innovators and activists in oral health and other sectors, as well as promote the GOHL Institute objectives to foster a global community of practice, lead system change through evidence and innovation, empower transformational leadership, bridge sectors, and catalyze cross-sectoral impact, to interested collaborators. The event kicked off with two panel discussions led by Dr. Marko Vujicic, Chief Economist and Vice President of the Health Policy Institute, American Dental Association, and Dr. Greg Chadwick, President of the FDI World Dental Federation and Dean of the East Carolina University School of Dental Medicine, where they addressed evidence-based advocacy and policy in the face of challenging contexts, interdisciplinary collaboration to achieve system-level change, and actionable steps through innovation and sustainability.</p><p>“I feel we’re at the beginning of a ‘Golden Era’ of oral health internationally. A decade ago, we did not have compelling evidence that investing in oral health, for example, saves money on pregnancy outcomes or reduces healthcare costs among people with diabetes. There is a very strong value proposition for oral health today that we didn’t have a decade ago. There’s significant momentum, and a lot of it has to do with economics. The fact that we can show there is a financial ROI to investing in oral health is really important in policy circles,” Dr. Vujicic said during his keynote. “This ‘Golden Era’ opportunity for oral health is inconsistent with the status quo financing and delivery models that exist. So that is the tension I feel the leaders of the profession need to address, a fundamental realignment in what oral health care focuses on.”</p><p>The program culminated with a dinner, enabling dialogue between changemakers in oral health. In January 2024, I was approached by a couple of companies who wanted to get my opinion on their 3D printing system.&nbsp; I had worked with the tech before, but I was interested to see how it had evolved.&nbsp; As I began to tinker and experiment with those systems, I was blown away at what was possible.<p></p><p>Ever since then, I've worked on other projects, but I've always come back to 3D printing.&nbsp; &nbsp;Why, you may ask?&nbsp; Because what you can do is flat out amazing.&nbsp; I remember when I first saw 3D printing at a dental meeting.&nbsp; It was at the ADA meeting in Washington, DC.&nbsp; That would have been probably 2016 or 2017.&nbsp; It was amazing even then, but things have changed so dramatically since then.&nbsp; Basically, if you haven't seem 3D printing for dentistry lately, you're missing a lot.</p><p>When CBCT first started to go mainstream in the profession around 2008 we saw the very beginning of what 3D could do.&nbsp; Now with the amazing progress we've had with intraoral scanning and AI, we're perched on the edge of dentistry's next big thing.&nbsp; We're going to see 3D patient info merge into all kinds of systems that will work together to give us better patient outcomes than we ever thought possible.</p><p>Up until recently the problem with 3D printing was the design, but AI has made incredible inroads into that aspect of things.&nbsp; The ADA is exploring new meeting possibilities and programs to advance oral and overall health by keeping dental professionals up to date on the latest scientific guidelines and technical innovations. Further information will be shared when it is available.</p></blockquote><p>That means that the association will still continue to explore options for some type of meeting, which in my opinion is good news.&nbsp; Education is critical to the profession, and the ADA has frequently helped provide some of the best education there is in dentistry.&nbsp; I'm hopeful that whatever comes next in the required scientific session is successful.&nbsp; It will probably be smaller and not such an expensive line item in the budget, but that's ok by me.</p><p>More importantly, dentistry needs the united voice that is the ADA.&nbsp; There is a huge benefit to the profession speaking with a single voice, and that voice is the ADA.&nbsp; Dentistry has benefitted greatly from the hard work of the ADA.&nbsp; Now they can take the financial resources that were being poured into the meeting and use it for other projects.&nbsp; I'm hopeful of what they can with that cash.</p><p>Here's a bit of nostalgia... in 1998 what was then called "The ADA Annual Session" was flying high.&nbsp; This was in the very early days of the Internet and computers in healthcare.&nbsp; Back then I had started taking computers apart in my office and had begun writing for a newsletter (photocopied and delivered via USPS) called 'Tools R Us' and published by a good friend Dr. Bob Davis.</p><p>Back then my writing for Bob's newsletter on technology in dentistry had somehow come to the attention of some folks at the ADA headquarters in Chicago.&nbsp; With technology poised to have a major impact on the healthcare industry, the ADA had decided to have a special day on the subject as part of their Annual Session.&nbsp; The first ever ADA Technology Day was in San Francisco in 1998.</p><p>I vividly remember my first lecture for the ADA.&nbsp; One of the things I spoke about was a new technology that allowed users to connect peripheral devices to their computers.&nbsp; It was called "Universal Serial Bus" and was abbreviated USB.&nbsp; I clearly remember talking about it while audience members furiously scribbled in their notebooks.&nbsp; There were some vendors displaying leading edge tech and the one thing I was really blown away by was a 17 inch flat screen display being shown by Intel.&nbsp; I had seen pictures of them, but this was my first time to actually see one in person.&nbsp; The retail price was $7000!</p><p>I was really lucky.&nbsp; At that meeting I made a lot of good industry contacts, which is important for anyone who wants to educate their peers.&nbsp; I spoke at every ADA meeting for about the next 25 years.&nbsp; Often getting into the show meant going through the doors in a mass of humanity.&nbsp; It was incredible.&nbsp;&nbsp;</p><p>As someone who has made a career out of being in love with tech I can tell you without a doubt, everything evolves and as progress is made older ideas and tools are replaced.&nbsp; The evolution of broadband connections and video has changed the way many people receive their information.&nbsp; I still think some in-person meetings will survive, but we've now entered a time when we are going to see contraction in their numbers.&nbsp; The ADA Annual Session was a real boon to my career as Technology Evangelist and I'm sad to see it go.&nbsp; However it makes no sense to spend resources on a meeting that few attend.&nbsp; At the end of the day, this was a business decision.&nbsp; I get that and I support the decision, but I can still be nostalgic for those good old days.</p><p><a href="https://www.ada.org/about/press-releases/final-smilecon-to-be-held-in-washington-dc-in-october-2025" target="_blank">The ADA press release can be found here.</a>&nbsp; There was also a <a href="https://adanews.ada.org/ada-news/2025/may/ada-to-end-smilecon-in-2026/" target="_blank">post in the ADA News that you can read here.</a>&nbsp;&nbsp;</p> John Flucke - Technology Evangelist ADA Just Announced 2025 Smilecon Will be the LAST One!
