What follows is a list of senior living operators worth keeping an eye on in 2024: </p> <h3 class="wp-block-heading" id="h-lutheran-senior-services-nbsp">Lutheran Senior Services </h3> <p>St. Louis-based Lutheran Senior Services (LSS) has been on the move this year, most recently by announcing its<a href="https://seniorhousingnews.com/2024/03/29/lutheran-senior-services-diakon-plan-merger-in-bid-for-greater-scale/"> intent to join forces with Diakon</a>, a Lutheran-aligned senior living operator based in Pennsylvania.</p> <p>The deal is slated to create a new senior housing and care organization with a footprint of one dozen life plan communities, one assisted living community and nine affordable senior housing communities. Going by the most recent LeadingAge Ziegler 200 report, that would make LSS the 12th-largest nonprofit senior housing operator in the country.</p> <p>“[The deal] will make sure that we are better able to compete for talent; make sure that we can continue to add and to grow specialty positions within the organization; create career-ladder opportunities for our team; and then obviously, increase purchasing power, negotiating power, spread overhead, improve access to capital,” LSS CEO Adam Marles told Senior Housing News in March.</p> <p>Marles and the organization are not done growing and evolving this year, either. He previously told SHN that the organization has a new strategic framework and is engaged in a “pivot year” in 2024. That pivot includes moving away from skilled nursing and deeper into core private-pay senior living services such as independent living, assisted living and memory care.</p> <p>“For us a laser focus right now is making sure that we have sustainable communities for the long-term, recognizing that boomers’ demands are going to be so much different than our current consumer,” Marles told Senior Housing News.</p> <p>The organization’s focus on evolving for a new demographic of older adults, plus its fast and continued growth in 2024, make LSS one worth keeping an eye on this year.</p> <h3 class="wp-block-heading" id="h-retirement-unlimited-inc">Retirement Unlimited, Inc.</h3> <p>Retirement Unlimited, Inc. (RUI) is a company that’s likely already on the radar for some senior living industry watchers. But the operator has become an even more important one to keep an eye on in the last 12 months.</p> <p>In late December, RUI <a href="https://seniorhousingnews.com/2024/01/04/why-to-expect-more-deals-like-the-rui-brandywine-combination/">announced the acquisition</a> of Brandywine Living, a deal that increased the company’s portfolio to 59 communities across the country. With the acquisition of Brandywine, RUI picked up new scale in New Jersey, Connecticut, Delaware, Maryland, New York and Pennsylvania.</p> <p>The Roanoke, Virginia-based senior living operator has been on a fast growth trajectory since 2021, when it had just 12 communities in its portfolio. And in 2024, the company has gone on a hiring spree by adding regional-level leaders across the country.</p> <p>The company has grown in recent years with the help of real estate investment trust Welltower (NYSE: WELL), and in 2023, that relationship <a href="https://seniorhousingnews.com/2023/02/15/welltower-names-advisor-for-self-managed-operating-platform-announces-new-luxury-brand-with-rui/">helped fuel the launch</a> of a new luxury brand for RUI. Welltower CEO Shankh Mitra has also identified RUI as a top operating partner for the REIT.</p> <p>Helping to lead RUI is President Dorie-Ellis Sullivan, a seasoned senior housing industry leader with a clear vision for the company’s future in a fast-changing industry. </p> <p>Sullivan represents the next generation of industry leadership, and in 2023, she earned a spot as a Senior Housing News Changemaker. At the time, she spoke about the need for operators to evolve for the incoming baby boomer generation.</p> <p>“Our customer coming through the doors is completely different,” Sullivan told SHN in 2023 as part of that interview. “Going through customer-journey mapping, looking at other industries – I think senior living needs to focus on that.”</p> <p>In 2024, REITs and other deep-pocketed companies are in the driver’s seat with regard to new growth, and RUI’s Welltower backing will likely keep its lane for future expansion wide open. That reason, coupled with the company’s leadership, makes RUI a senior living operator to watch this year.</p> <h3 class="wp-block-heading" id="h-onelife-senior-living">Onelife Senior Living</h3> <p>Onelife Senior Living<a href="https://seniorhousingnews.com/2024/02/27/ally-senior-living-onelife-merge-forming-19-property-operator/"> recently merged</a> with Ally Senior Living and created a 19-community senior housing operator and management company in February, with headquarters in Denver, Colorado. </p> <p>The combined company was born out of Onelife Founder Zack Falk and former Ally CEO Dan Williams&#8217; shared vision for what the future of the senior living industry looks like. Both found vision and mission alignment and both shared the challenges of running a small to midsize operator during the Covid-19 pandemic. </p> <p>“You need some scale in order to get the resources to make the building successful and I think both Zack and I realized that and I think the other, smaller companies may be in the same position.”</p> <p>Now with approximately 1,500 employees and will focus on value-add acquisitions from distressed properties, Onelife will focus 2024 on integration of systems and teams of the newly-merged companies with the goal of three acquisitions by the year&#8217;s end. The company recently completed an acquisition of a community in Henderson, Nevada in the Las Vegas metro area. </p> <p>“The main goal is to get all that sorted out going into next year we’re really well-aligned with all of our systems and how we operate so that we can be a best-in-class operator. They typically span 30,000 square feet of amenity space.</p> <p>According to the REIT, the Affinity portfolio carries margins close to 60%, and there are opportunities to further expand it by three or four new properties a year. </p> <p>Looking ahead, wellness-focused properties for low-acuity residents will represent a bigger part of the REIT’s strategy, with companies like Affinity Living Group potentially at the forefront of that strategy. That, along with the active adult sector’s still-popular status among investors and owners, makes the company worth paying attention to this year.</p> <h3 class="wp-block-heading" id="h-gardant-management-solutions">Gardant Management Solutions</h3> <p>The senior living industry is at the cross-current of two major trends: The need to offer middle-market rates for residents, and the need to educate prospects about what senior living is in light of new scrutiny on the industry.</p> <p>Bourbonnais, Illinois,-based Gardant Management Solutions sits at the nexus of both trends. </p> <p>Thanks to the <a href="https://seniorhousingnews.com/2023/08/23/gardant-management-adds-25-senior-living-communities-to-growing-portfolio/">addition of about two-dozen former Pathway to Living communities to its portfolio</a> in 2023, Gardant is now one of the largest assisted living providers in the country with 80 communities. The effort could help researchers more broadly understand whether certain kinds of people are more likely to develop the disease over time.</p> <p>Funded by the National Institute on Aging, the study found that conscientiousness, extraversion and positive affect were associated with a lower risk of dementia, while those with negative affect and neuroticism, the study found, could have a higher risk. </p> <p>The results of the study were published in Alzheimer&#8217;s and Dementia, the Journal of the Alzheimer&#8217;s Association. THe study was conducted by Northwestern University researchers and the University of California Davis that analyzed data from eight longitudinal studies and over 44,500 participants. That has largely been a mistake, according to Dr. Karl Herrup. </p> <p>Herrup, who is a professor and investigator with the University of Pittsburgh’s Alzheimer’s Disease Research Center, thinks this is an issue that began back in 1906 when the first hypothesis regarding the disease was made.</p> <p>“To pursue it doggedly now, all these years later, and to use it as a rationale to try … to suppress the pursuit of other forms of research that might have looked for other causes, I think is where we&#8217;ve gone substantially wrong,” Herrup told Memory Care Business.</p> <p>Herrup has long studied what he considers flaws in the research and treatment of Alzheimer’s disease. It was the topic of his 2021 book, “How Not to Study a Disease: The Story of Alzheimer&#8217;s.” He also recently <a href="https://news.uchicago.edu/where-has-alzheimers-research-gone-wrong">spoke about the issue</a> on a University of Chicago podcast.</p> <p>To Herrup, “the best way to study Alzheimer’s disease is not to study Alzheimer’s disease,” as he recently said on the podcast. By that, he means researchers have been too focused on Alzheimer’s supposed amyloid plaque origins, and not on its potentially deeper, more nuanced origins. </p> <p>According to Herrup, a cross-section of older adults after death likely would reveal that many had high levels of plaque in their brains that would otherwise indicate Alzheimer’s disease, without ever having symptoms while they were alive. That is a curious result and worth more study and examination, he said.</p> <p>“We&#8217;re still spending an inordinate amount of time pursuing the amyloid basis of Alzheimer&#8217;s disease, and that&#8217;s just holding us back,” Herrup said. </p> <p>Instead, he is coming around to the notion that Alzheimer’s is a disease with many different smaller origins, inflammation being a big one. </p> <p>Research has shown benefits in lowering the risk for Alzheimer’s disease through lifestyle changes including managing blood pressure, mental and physical exercise and keeping blood sugar under control. </p> <p>“For both scientists and pharmaceutical people, I&#8217;m afraid we&#8217;ve got to deal with the complexity that none of us would have hoped for in the beginning,” he said. “For the public … it’s a complicated calculation to make, and it&#8217;s not remunerative for the industry.”</p> <p>Despite the issues he has noted, Herrup added the industry as a whole is starting to move in a “broader strategy” with how it is approaching the disease, specifically highlighting there is more interest in inflammation control and “many other potential causes.” </p> <p>An additional frustration Herrup noted is that when other research projects are funded, such as looking at DNA damage or aging and the impact of Alzheimer’s, amyloid plaque deposition is used “as a readout” to measure the success of the approach if the pathological markers are reduced. In doing so, it’s a “classic case of circular logic.”</p> <p>However, despite these frustrations, Herrup said he is optimistic about the future of Alzheimer’s disease research, and there is more traction these days for a non-amyloid approach. Additionally, he added there needs to be a better understanding of the biology of aging. </p> <p>“The future of Alzheimer&#8217;s research I think is still governed to a large extent by the orthodoxy,” he said. “So we will see a lot of studies about using anti-amyloid therapies in combination with other approaches.”</p> <p>He added: I&#8217;m speaking as a basic scientist, not not as a clinician, or as a drug development person, where there&#8217;s a certain urgency in bringing a product to market … I think we&#8217;re seriously underinvesting in improving our understanding of the biology of aging.”</p> <p>The solution can’t be a simple one, he noted, and it will be unlikely to be so.</p> <p>“Alzheimer&#8217;s is a hugely complicated disease,” Herrup said. “What we need from people is the patience to stay the course and let us keep banging away at this problem. I do think progress is happening. I do think it&#8217;s possible to really make an impact. Inspiration also came from the surrounding countryside, leading to carpet inspired by “old-growth forest floor,” a post-prohibition secret door leading to the dining room, and other carpets inspired by sand and ocean tones to capture the nearby beaches.</p> <p>In order to meet the resident demand for more interaction opportunities, the renovations included opened floor plans and created “intimate gathering areas” within the space. The company also offers home care services and has three majority-owned PACE properties. </p> <p>Total revenue for the company stood around $175 million in 2023, and looking ahead, Chief of Residential Services Nicole Maag said Brio Living Services has a &#8220;financial roadmap&#8221; crafted by CEO Steve Fetyko for the company&#8217;s future bottom-line success. </p> <p>&#8220;It allows us now to go into being proactive versus being in that reactive mode,&#8221; Maag said during an interview with SHN at Senior Living 100 in Laguna Niguel, California. &#8220;Our focus is to build-out as fast as we can.&#8221; </p> <p>The company is currently expanding independent living at select properties and has relaunched its Avenues home care program that was shuttered during the Covid-19 pandemic. </p> <p>Brio Living Services is also piloting programming and branded lifestyle offerings at its communities in order to inform a wider rollout down the road, Maag said. That rollout is slated to include a community paramedic program and a new wellness and lifestyle brand called Balanced Living. Brio’s leaders also are focused on expanding the organization’s PACE footprint and adding affordable housing units at those locations in the future. </p> <p>The expansion into independent living has been driven by demand for an &#8220;in-between&#8221; IL model that will include approximately 20 units to create a &#8220;hub&#8221; within the community for a new independent living product, Maag said. The latest story, published Monday, centered mainly on a staffing algorithm used by Brookdale Senior Living (NYSE: BKD) known [&#8230;]</p> <p>The post <a href="https://seniorhousingnews.com/2024/04/01/new-washington-post-investigation-centers-on-brookdale-senior-living-staffing-algorithm/">New Washington Post Investigation Centers on Brookdale Senior Living Staffing Algorithm</a> appeared first on <a href="https://seniorhousingnews.com">Senior Housing News</a>.</p> <p>Staffing algorithms like one used by the nation&#8217;s largest senior living operator are exacerbating work shortages and often underestimating the amount of care that older adults need, according to a new <a href="https://www.washingtonpost.com/business/2024/04/01/assisted-living-algorithm-staffing-lawsuits-brookdale/">investigation</a> from the Washington Post.</p> <p>The latest story, published Monday, centered mainly on a staffing algorithm used by Brookdale Senior Living (NYSE: BKD) known as “Service Alignment.” According to the Post, the algorithm helped lead to understaffing at the company’s communities, and in the process “failed to capture the nuances of caring for vulnerable seniors.”</p> <p>“When Service Alignment was rolled out, the algorithm forced many Brookdale facilities to reduce their staff, documents and interviews show. At other facilities, where the algorithm suggested more staff was needed, Brookdale intervened,” wrote Post reporters Douglas MacMillan and Christopher Rowland.</p> <p>The Post’s reporters based their story on interviews with more than 35 current and former Brookdale employees, court records and internal documents.</p> <p>In a statement provided by a Brookdale spokesperson, the Brentwood, Tennessee-based company disagreed with the Post&#8217;s account.</p> <p>“Service Alignment is a resource to help guide community leaders on staffing decisions, but local leaders have autonomy and discretion to adjust staffing levels as necessary to meet resident needs. It was created in the early 2000s as a way to identify best practices to meet the needs of residents. Service Alignment does not dictate staffing maximums or determine care costs,” the company’s statement read.</p> <p>The Post’s investigation on Brookdale’s staffing algorithm is the latest in a series of stories called “Memory Inc.” casting a spotlight on the U.S. assisted living sector. A previous story, <a href="https://seniorhousingnews.com/2023/12/17/washington-post-highlights-patterns-of-resident-elopement-dangerously-understaffed-senior-living-communities/">published</a> in December, 2023, centered on thousands of cases since 2018 where senior living residents wandered away from their communities, resulting in nearly 100 deaths in that time. </p> <p>That story sent shockwaves through the industry shortly after it was published, and <a href="https://seniorhousingnews.com/2024/01/25/gardant-co-president-industry-associations-call-for-better-public-private-collaboration-in-senate-hearing/">prompted</a> a U.S. Senate committee hearing in January.</p> <p>Brookdale is the largest senior living operator in the U.S. with 652 communities nationwide.</p> <h3 class="wp-block-heading" id="h-algorithm-story-adds-to-industry-scrutiny">Algorithm story adds to industry scrutiny</h3> <p>According to the Washington Post, Brookdale’s staffing algorithm has led to consternation at the community level. Managers “repeatedly complained that the company’s algorithm underestimated the amount of labor they needed to meet resident needs, according to court records,” the Post story noted.</p> <p>Sometimes, the algorithm led to substandard care for older adults with dementia, the Post found after talking with a handful of the company’s former executive directors.</p> <p>“Service Alignment failed to capture the complexities of working with seniors with cognitive decline,” the Post reporters wrote. “For instance, helping dementia patients take showers may take two or three times as long as other residents, because they often refuse to get undressed in front of caregivers they may not remember and need to be patiently guided through the process.”</p> <p>The Post also detailed two civil lawsuits against Brookdale in Tennessee and California in which “a dozen residents or relatives of residents claim they suffered due to short-staffing caused by an overreliance on algorithms.” Sunrise Senior Living also is defending itself in a lawsuit by a group of customers with &#8220;similar concerns,&#8221; according to the Post.</p> <p>Brookdale told the Post the claims were “inaccurate.” Brookdale also said that the staffing allegations are “largely derived from purported class action litigation that we and a number of other senior living providers have been facing over the last several years driven by plaintiffs’ attorneys seeking to earn fees.” </p> <p>“Brookdale disputes the allegations and has been successful in defending against them to date,” the company’s statement read.</p> <p>Brookdale is not the only company that has undergone legal and media scrutiny in the last 12 months over use of an algorithm. The UnitedHealth Group (NYSE: UNH) is <a href="https://skillednursingnews.com/2023/11/unitedhealth-faces-lawsuit-media-scrutiny-over-ais-role-in-medicare-advantage-denials-of-nursing-home-care/">facing a federal lawsuit</a> over UnitedHealthcare’s use of an artificial intelligence tool in making Medicare Advantage claims denials for patients in extended care facilities.</p> <p>In general, the senior living industry is facing increased scrutiny from multiple sides in 2024, including from the media and local and federal regulators. The result created the nation&#8217;s fifth largest senior housing operator, and also followed a<a href="https://seniorhousingnews.com/2022/10/12/discovery-recaps-with-coastwood-lee-equity-decker-named-board-chair/"> recapitalization</a> of Discovery Senior Living by Coastwood in October of 2022.</p> <p>The recapitalization added fuel to the fire for Discovery&#8217;s horizontal growth strategy, and in August of last year, resulted in the<a href="https://seniorhousingnews.com/2023/08/11/discovery-senior-living-launches-new-management-company-with-regional-footprint/"> launch of LakeHouse Senior Living</a>, a new regional management company aimed at growing the brand&#8217;s presence in the Midwest. </p> <h3 class="wp-block-heading" id="h-scale-support-from-discovery-a-game-changer-for-isl">Scale, support from Discovery a ‘game-changer&#8217; for ISL</h3> <p>Now Discovery’s largest, vertically integrated senior living operator, ISL exists within the Discovery family of companies, with ISL having management of over 113 communities. </p> <p>ISL is continuing to grow its brand across its footprints in 24 states. As president and CEO of ISL, Gray continues to lead the Carlsbad, California-based organization while also sitting on the Discovery Senior Living board of trustees. </p> <p>As a result of the integration, some ISL teams have been elevated to the corporate level to support communities outside of the ISL portfolio, with an in-house recruitment team now providing insights to communities for Discovery and beyond, Gray said. </p> <p>&#8220;We&#8217;re taking the good things that are happening from both companies and building them out and growing them so our support services platform is robust and I would say it&#8217;s one of the biggest things to have happened as a result of this transaction,&#8221; Gray told SHN. </p> <p>The new support has been a &#8220;game changer&#8221; for ISL&#8217;s operations, specifically for HR-related tasks, Gray said. The company has improved employee retention and recruitment by enacting an improved 401(k) program for staff with an employer match. </p> <p>&#8220;As we are growing and as we are bringing all of those things into the parent company, our staff can focus on the day-to-day and deliver the resident experience and drive results,&#8221; Gray said. </p> <p>On Monday, Gray confirmed that long-time Brookdale Senior Living (NYSE: BKD) executive Laura Fischer has started in her new role as COO for ISL. According to her profile on the Brookdale website, Fischer was previously the Division Vice President for Brookdale&#8217;s West division, joining Brookdale in 2017. She brings over two decades of senior living experience to her new role with ISL. </p> <p>The ecosystem of Discovery&#8217;s deep bench of executive leadership across its regional brand landscape has allowed ISL to find best practices in operations and glean new ideas to improve resident and employee experiences. </p> <p>&#8220;We&#8217;re able to talk about best practices and find out what&#8217;s working and it&#8217;s a different way to grow,&#8221; Gray said. </p> <h3 class="wp-block-heading" id="h-improving-operations-to-evolve-in-2024">Improving operations to evolve in 2024</h3> <p>While operators remain challenged by headwinds in new development this year, many have found new ways to grow in 2024, from campus redevelopment projects to acquiring and turning around communities at a discount to replacement costs. ISL has remained somewhat active on development based on the assumption that demand in the future will ease the headaches of pushing through construction in today&#8217;s higher-interest climate. </p> <p>For 2024, Gray said ISL would continue to find &#8220;operational excellence&#8221; as the integration continues with Discovery. With the move, LSS is adding four life plan communities to its portfolio, which will encompass a dozen life plan communities, one assisted living community and nine affordable senior housing communities when all is said and done.</p> <p>Marles, who has led St. Louis-based LSS since 2021, said the decision was driven partly by a belief that “scale is going to matter for the success of non-profit senior living moving forward.”</p> <p>“It will make sure that we are better able to compete for talent; make sure that we can continue to add and to grow specialty positions within the organization; create career-ladder opportunities for our team; and then obviously, increase purchasing power, negotiating power, spread overhead, improve access to capital,” Marles told Senior Housing News.</p> <p>The newly combined organization would have a total of more than 3,500 team members with more than 5,300 residents and rank as the 12th largest non-profit senior living operator, at least according to the most recent LeadingAge Ziegler 200 report, <a href="https://seniorhousingnews.com/2023/09/22/national-senior-communities-good-sam-presbyterian-homes-top-list-of-largest-senior-living-nonprofits/">released</a> last year. </p> <p>Factors going into the agreement, according to Marles, includes Diakon’s long term “legacy of serving their residents well” and the financial stability of both organizations</p> <p>“This is a transformative moment in the 165 year history of Lutheran Senior Services,” Marles said. “We collectively are really excited about what this will mean for our organization moving forward and set us on a trajectory to continue our growth, continue to improve all of our services, while maintaining our our quality and and our ability to care not only for the people we serve, but also the employees of LSS.”  </p> <p>Diakon-managed communities are keeping their respective names in the merger. The organizations expect to finalize the affiliation later this summer.</p> <p>The announcement follows LSS’ ongoing “<a href="https://seniorhousingnews.com/2024/01/10/pivot-year-how-lutheran-senior-senior-services-is-shifting-for-new-generation-of-residents/">pivot year</a>” in 2024, In January, LSS acquired St. Louis Home Health, which Marles said added an orthopedic specialty to the organization’s home health line and brought LSS home health services into Illinois. In February, LSS acquired New Horizons PACE, a Program of All-Inclusive Care for the Elderly (PACE) program in the St. Louis region.</p> <p>“Home health, PACE and life plan communities all serve people in different types of ways, and we believe that diversification of services is going to be critical to our long term success,” Marles said.</p> <p>Additionally, in 2022 LSS created a new strategic framework for the organization with four primary goals; culture that “brings out the best in each other,” products and services that evolve with changing consumer preferences, growth through partnership and collaboration and a sustainable financial future.
