5 Senior Living Operators to Watch in 2024

Steady recovery amid ever-changing challenges, new operating models to spur innovation, consolidation and partnerships— these are some of the traits of senior living organizations standing out in 2024.

Companies are pushing through headwinds and their work is worth tracking. What follows is a list of senior living operators worth keeping an eye on in 2024:

Lutheran Senior Services

St. Louis-based Lutheran Senior Services (LSS) has been on the move this year, most recently by announcing its intent to join forces with Diakon, a Lutheran-aligned senior living operator based in Pennsylvania.

The deal is slated to create a new senior housing and care organization with a footprint of one dozen life plan communities, one assisted living community and nine affordable senior housing communities. Going by the most recent LeadingAge Ziegler 200 report, that would make LSS the 12th-largest nonprofit senior housing operator in the country.

“[The deal] will make sure that we are better able to compete for talent; make sure that we can continue to add and to grow specialty positions within the organization; create career-ladder opportunities for our team; and then obviously, increase purchasing power, negotiating power, spread overhead, improve access to capital,” LSS CEO Adam Marles told Senior Housing News in March.

Marles and the organization are not done growing and evolving this year, either. He previously told SHN that the organization has a new strategic framework and is engaged in a “pivot year” in 2024. That pivot includes moving away from skilled nursing and deeper into core private-pay senior living services such as independent living, assisted living and memory care.

“For us a laser focus right now is making sure that we have sustainable communities for the long-term, recognizing that boomers’ demands are going to be so much different than our current consumer,” Marles told Senior Housing News.

The organization’s focus on evolving for a new demographic of older adults, plus its fast and continued growth in 2024, make LSS one worth keeping an eye on this year.

Retirement Unlimited, Inc.

Retirement Unlimited, Inc. (RUI) is a company that’s likely already on the radar for some senior living industry watchers. But the operator has become an even more important one to keep an eye on in the last 12 months.

In late December, RUI announced the acquisition of Brandywine Living, a deal that increased the company’s portfolio to 59 communities across the country. With the acquisition of Brandywine, RUI picked up new scale in New Jersey, Connecticut, Delaware, Maryland, New York and Pennsylvania.

The Roanoke, Virginia-based senior living operator has been on a fast growth trajectory since 2021, when it had just 12 communities in its portfolio. And in 2024, the company has gone on a hiring spree by adding regional-level leaders across the country.

The company has grown in recent years with the help of real estate investment trust Welltower (NYSE: WELL), and in 2023, that relationship helped fuel the launch of a new luxury brand for RUI. Welltower CEO Shankh Mitra has also identified RUI as a top operating partner for the REIT.

Helping to lead RUI is President Dorie-Ellis Sullivan, a seasoned senior housing industry leader with a clear vision for the company’s future in a fast-changing industry.

Sullivan represents the next generation of industry leadership, and in 2023, she earned a spot as a Senior Housing News Changemaker. At the time, she spoke about the need for operators to evolve for the incoming baby boomer generation.

“Our customer coming through the doors is completely different,” Sullivan told SHN in 2023 as part of that interview. “Going through customer-journey mapping, looking at other industries – I think senior living needs to focus on that.”

In 2024, REITs and other deep-pocketed companies are in the driver’s seat with regard to new growth, and RUI’s Welltower backing will likely keep its lane for future expansion wide open. That reason, coupled with the company’s leadership, makes RUI a senior living operator to watch this year.

Onelife Senior Living

Onelife Senior Living recently merged with Ally Senior Living and created a 19-community senior housing operator and management company in February, with headquarters in Denver, Colorado.

The combined company was born out of Onelife Founder Zack Falk and former Ally CEO Dan Williams’ shared vision for what the future of the senior living industry looks like. Both found vision and mission alignment and both shared the challenges of running a small to midsize operator during the Covid-19 pandemic.

“You need some scale in order to get the resources to make the building successful and I think both Zack and I realized that and I think the other, smaller companies may be in the same position.”

Now with approximately 1,500 employees and will focus on value-add acquisitions from distressed properties, Onelife will focus 2024 on integration of systems and teams of the newly-merged companies with the goal of three acquisitions by the year’s end. The company recently completed an acquisition of a community in Henderson, Nevada in the Las Vegas metro area.

“The main goal is to get all that sorted out going into next year we’re really well-aligned with all of our systems and how we operate so that we can be a best-in-class operator. We’re not going to let the integration process stop us from growth,” Williams said during a recent SHN Transform podcast interview.

Onelife and Ally’s merger could be how smaller senior living operators might fuel growth by joining force, and for that reason, makes Onelife a provider to watch this year.

Affinity Living Communities

Affinity Living Communities gained new prominence earlier this year when it was labeled as one of the “major players” in the space by Welltower CEO Shankh Mitra, after the REIT acquired a 25-property portfolio managed by the company for nearly $1 billion in February.

President Darin Davidson has noted that Welltower is “a partner with shared values and forward thinking.”

“The Welltower partnership will enable us to enhance and extend our ability to execute our mission of creating thriving communities in which our engaged residents live full and happy lives,” he said in a press release at the time.

The 3,900-unit portfolio is mostly concentrated in the Pacific Northwest and expands Welltower’s “wellness housing portfolio.” The communities are purpose-built with an average age of eight years, with resident rates of about $2,100. They typically span 30,000 square feet of amenity space.

According to the REIT, the Affinity portfolio carries margins close to 60%, and there are opportunities to further expand it by three or four new properties a year.

Looking ahead, wellness-focused properties for low-acuity residents will represent a bigger part of the REIT’s strategy, with companies like Affinity Living Group potentially at the forefront of that strategy. That, along with the active adult sector’s still-popular status among investors and owners, makes the company worth paying attention to this year.

Gardant Management Solutions

The senior living industry is at the cross-current of two major trends: The need to offer middle-market rates for residents, and the need to educate prospects about what senior living is in light of new scrutiny on the industry.

Bourbonnais, Illinois,-based Gardant Management Solutions sits at the nexus of both trends.

Thanks to the addition of about two-dozen former Pathway to Living communities to its portfolio in 2023, Gardant is now one of the largest assisted living providers in the country with 80 communities. The company’s middle-market model utilizes a combination of private pay rates and Medicaid waivers to keep costs lower for residents.

Gardant currently serves about 6,000 older adults through the Medicaid waiver program, an effort that has required “an entirely different business model altogether,” Simpkins said.

The operator’s model was on full display earlier this year during a Senate hearing prompted by recent Washington Post articles on resident elopement, when Gardant Management Co-President and COO Julie Simpkins spoke about the need to offer middle-market options for residents.

“With a rapidly growing elderly population, we need a public and private partnership to incentivize more providers to develop these models,” Simpkins said at the Senate hearing. “From expanding more affordable long-term care options to workforce programs addressing the growing caregiver shortages, these efforts could make a real difference.”

Simpkins is not shy about the fact that she sees a greater role for federal and state governments in helping subsidize the cost of senior living – and she is not the only one in the industry who feels that way, either.

Given the company’s fast growth and innovation at the forefront of the public payment trend in senior living, Gardant Management is a senior living operator to watch in 2024.

The Aspenwood Company acquires four North Texas communities

The Aspenwood Company announced that it has acquired four retirement communities in North Texas operating under The Village on the Park brand.

According to a press release, the communities are located in Plano, Denton and McKinney, Texas, and consist of 336 units.

The release states the communities are situated in affluent neighborhoods and strategically positioned amidst areas “experiencing remarkable population growth.”

Management

The Aspenwood Company assumes management of two communities

The Aspenwood Company announced on April 1 that it added two communities in Houston and Dallas to its portfolio.

According to a press release, the communities are The Doliver of Tanglewood in Houston and Heartis Mid Cities in the Dallas metropolitan area.

“We are proud to be chosen to manage these two exceptional Texas communities, where residents and associates expect and deserve our unwavering commitment to excellence,” Heather Tussing, president of The Aspenwood Company said in the release. “We make it our mission to help our residents live life well.”

United Church Homes Management expands Columbus footprint

United Church Homes Management announced it has extended its partnership with two additional senior living communities in Columbus, Ohio and will be providing management services for them.

According to a press release, the communities are Bexley Heritage Apartments and Village Shalom Apartments.

“”We are excited to deepen our commitment to the Columbus community through our expanded partnership,” Dan Fagan, vice president of care and housing at United Church Homes said in the release. “We believe in providing enriching environments for older adults. With our continued management of Wexner Heritage Village and our new partnerships we are dedicated to upholding the highest standards in abundant living.”

Transactions

SLIB sells southeast Texas portfolio

Senior Living Investment Brokerage announced it facilitated the sale of a 237-bed portfolio consisting of an assisted living and a skilled nursing community in southeast Texas on April 1.

According to a press release, both communities are located within 25 miles of each other around the Bryan-College Station market.

The release states the seller is a non-profit Catholic hospital system, which was selling its only skilled nursing/seniors housing communities, and the buyer owns several skilled nursing facilities in Texas and is leasing the operations of these buildings to a group out of the Dallas-Fort Worth area.

Study: Positivity, Extraversion Linked to Lower Risk of Dementia

A recent study has linked personality traits with overall dementia risk. The effort could help researchers more broadly understand whether certain kinds of people are more likely to develop the disease over time.

Funded by the National Institute on Aging, the study found that conscientiousness, extraversion and positive affect were associated with a lower risk of dementia, while those with negative affect and neuroticism, the study found, could have a higher risk.

The results of the study were published in Alzheimer’s and Dementia, the Journal of the Alzheimer’s Association. THe study was conducted by Northwestern University researchers and the University of California Davis that analyzed data from eight longitudinal studies and over 44,500 participants. Out of those participants, 1,703 developed dementia and 50% of participants were subject to an autopsy following end of life.

According to an article published by NIA, the researchers “harmonized the data” and tested personality traits and reports of well-being that “predicted neuropsychological and neuropathological characteristics of dementia.”

Results from the study appear to show that certain personality traits could be warnings for dementia, including: Neuroticism, low conscientiousness, and negative affect while those who reported exhibiting conscientiousness, extraversion, and positive affect were less likely to develop dementia risk.

But, according to the article, a limitation of the study was the lack of non-representative participant data by race and researchers said more work was needed to connections using biomarkers of dementia and a broader, more diverse sample set.

Why One Expert Thinks Alzheimer’s Researchers Should Go Back to the Drawing Board

Alzheimer’s researchers have for decades focused much of their study on the amyloid plaques thought to cause the disease. That has largely been a mistake, according to Dr. Karl Herrup.

Herrup, who is a professor and investigator with the University of Pittsburgh’s Alzheimer’s Disease Research Center, thinks this is an issue that began back in 1906 when the first hypothesis regarding the disease was made.

“To pursue it doggedly now, all these years later, and to use it as a rationale to try … to suppress the pursuit of other forms of research that might have looked for other causes, I think is where we’ve gone substantially wrong,” Herrup told Memory Care Business.

Herrup has long studied what he considers flaws in the research and treatment of Alzheimer’s disease. It was the topic of his 2021 book, “How Not to Study a Disease: The Story of Alzheimer’s.” He also recently spoke about the issue on a University of Chicago podcast.

To Herrup, “the best way to study Alzheimer’s disease is not to study Alzheimer’s disease,” as he recently said on the podcast. By that, he means researchers have been too focused on Alzheimer’s supposed amyloid plaque origins, and not on its potentially deeper, more nuanced origins.

According to Herrup, a cross-section of older adults after death likely would reveal that many had high levels of plaque in their brains that would otherwise indicate Alzheimer’s disease, without ever having symptoms while they were alive. That is a curious result and worth more study and examination, he said.

“We’re still spending an inordinate amount of time pursuing the amyloid basis of Alzheimer’s disease, and that’s just holding us back,” Herrup said.

Instead, he is coming around to the notion that Alzheimer’s is a disease with many different smaller origins, inflammation being a big one.

Research has shown benefits in lowering the risk for Alzheimer’s disease through lifestyle changes including managing blood pressure, mental and physical exercise and keeping blood sugar under control.

“For both scientists and pharmaceutical people, I’m afraid we’ve got to deal with the complexity that none of us would have hoped for in the beginning,” he said. “For the public … it’s a complicated calculation to make, and it’s not remunerative for the industry.”

Despite the issues he has noted, Herrup added the industry as a whole is starting to move in a “broader strategy” with how it is approaching the disease, specifically highlighting there is more interest in inflammation control and “many other potential causes.”

An additional frustration Herrup noted is that when other research projects are funded, such as looking at DNA damage or aging and the impact of Alzheimer’s, amyloid plaque deposition is used “as a readout” to measure the success of the approach if the pathological markers are reduced. In doing so, it’s a “classic case of circular logic.”

However, despite these frustrations, Herrup said he is optimistic about the future of Alzheimer’s disease research, and there is more traction these days for a non-amyloid approach. Additionally, he added there needs to be a better understanding of the biology of aging.

“The future of Alzheimer’s research I think is still governed to a large extent by the orthodoxy,” he said. “So we will see a lot of studies about using anti-amyloid therapies in combination with other approaches.”

He added: I’m speaking as a basic scientist, not not as a clinician, or as a drug development person, where there’s a certain urgency in bringing a product to market … I think we’re seriously underinvesting in improving our understanding of the biology of aging.”

The solution can’t be a simple one, he noted, and it will be unlikely to be so.

“Alzheimer’s is a hugely complicated disease,” Herrup said. “What we need from people is the patience to stay the course and let us keep banging away at this problem. I do think progress is happening. I do think it’s possible to really make an impact. I would change the way we go about it, but I remain optimistic.”

3 Senior Living Projects That Showcase International Innovation

There is plenty of innovation in architecture and design occurring in the U.S.-based senior living industry. The same is true across the globe.

Forward-thinking architecture and design concepts were on display in three projects submitted to the 2023 Senior Housing News Architecture and Design Awards: Royal View by Auria Senior Living, Kunz Raubenheimer Architects and Mike Buyskes Construction in Johannesburg, South Africa; Revera Hollyburn House, by Revera Retirement Living, Kasian Architecture, Interior Design and Planning and Scott Constructions, in West Vancouver, Canada; and Guildwood Retirement Residences by Chartwell Retirement Residences, Michael Spaziani Architect Inc. and Collaborative Structures Limited in Ontario, Canada.

Each entry took a unique design to their approach, whether it was the introduction of modern contemporary architecture, biophilic design principles or embracing local architecture to make better uses of space.

First place: Royal View by Aria Senior Living

Completed Jan. 6, 2023, the Royal View by Aria Senior Living comprises apartments of “various configurations with sweeping views” overlooking the Royal Johannesburg Golf Course.

The community’s designers utilized contemporary architecture stylings to create a sense of space in the densely populated residential zone where the Royal View community is located.

“The development floats above the most beautiful golf course and creates a sense of wanting to get out and enjoy the outdoors,” the document states. “Now that the development is operational, we are delighted to see residents readily spilling onto the golf course, walking daily, and enjoying nature.”

The community has a variety of features for residents and offerings the general public can utilize as well, highlighted by recreational areas that “flow onto a deck overlooking the 13th green” of the neighboring golf course. In those areas, residents can grab a coffee, enjoy fine dining and a full lounge.

Royal View also boasts a fully integrated wellness center and clubhouse are available for the public to utilize for physiotherapy and biokineticists services.

Among the other noted amenities for the assisted living and memory care offerings is the integrated basement parking for residents in an effort to keep residents close to elevators when they need to leave.

“We have ensured careful integration of independent living, the wellness center as well as a full care center with assisted living and dementia care, all on the same property,” the submission states.

Ric D’Amico, senior vice president of Real Estate and Development and judge for the international award category, wrote he was delighted by Royal View’s design of open spaces.

“[They] allowed an abundance of light into the interiors and a natural connection to nature, throughout the environment,” D’Amico wrote. “I also like the redundancy on the exterior facade.”

Fellow judge Melissa Pritchard, managing principal of SFCS Architects, wrote the project is “modern and well executed that connects well to the natural setting of the golf course.”

Second place: Revera Hollyburn House by Revera Retirement Living

Completed Nov. 1, 2022, the renovations at the Revera Hollyburn House in West Vancouver, Canada were needed to better utilize the common areas, with the company noting they were previously “dysfunctional.”

In order to best determine what was needed, Revera Retirement Living formed groups to survey potential residents and stakeholders to “capture the needs of residents and staff through every milestone.”

“As we approached the renovation, it was critical to embody the local architecture, culture and landscape,” the document states.

The leading inspiration for the renovated building came from the nearby Lions Gate Bridge, and incorporated etched bridge cable patterns in the millwork and columns. Inspiration also came from the surrounding countryside, leading to carpet inspired by “old-growth forest floor,” a post-prohibition secret door leading to the dining room, and other carpets inspired by sand and ocean tones to capture the nearby beaches.

In order to meet the resident demand for more interaction opportunities, the renovations included opened floor plans and created “intimate gathering areas” within the space. Additionally, access, flow and utilization were all improved once the renovations were completed.

“Renovations have cultivated new life in the lobby and lounge areas as Hollyburn House staff and residents continue to thrive in their renewed space,” the document states. “With the West Vancouver culture now woven throughout the design, communal areas are a daily draw for residents and families.”

D’Amico noted his appreciation of the hospitality program and a seamless transition between public spaces gives the community very high marks.

Pritchard noted she appreciated the improvements and the intent to connect with nature, though noted the dining results were still on the dark and heavy side.

Third place: Guildwood Retirement Services by Chartwell Retirement Services

With the project completed Jan. 17, 2022, the Guildwood Retirement Services community sought to be a “beacon of innovation and thoughtful design.”

To do that, the Chartwell community’s design team incorporated elements of biophilic design by utilizing inspiration from the natural world, green roof plantings, a connection to the surrounding history and offering rooftop views.

“Characterized by its clean lines, organic color palette and love for different materials, the Chartwell Guildwood pays homage to this iconic design era,” the document states.

A standout feature for the luxury community is its rooftop lounge, which features “panoramic views” of Lake Ontario. Additionally, the community has a spa with a heated indoor pool along with a fitness center to “cater to holistic wellness.”

Alongside the community’s craftsmanship, it boasts mid-century modern elements to offer a “unique, nostalgic and contemporary style.”

The reception to the community has reportedly been positive, and it opened with a lease rate of 97%.

Cost information was not available for this project.

D’Amico wrote the project was impressive with a “great design intent that is visible in the final project,” though he noted he found the main entry sequence to be “stark and non-welcoming.”

Pritchard noted she also wasn’t keen on the entry sequence, but the integration of the surrounding community was well done.

“Once within the building the use of a very wide color palette, the choice of materials and the texture created by the interior team is outstanding,” Pritchard wrote.

Presbyterian Senior Living promotes COO

Presbyterian Senior Living announced the promotion of Kate Hershey to chief operating officer on April 2.

Health Dimensions Group names senior director of recruitment and placement services

Health Dimensions Group has announced the promotion of Alex Knight as its senior director of recruitment and placement services.

In his role, Knight will oversee a team of recruiters and consultants in delivering workforce solutions to HDG and its clients.

Knight has been with HDG since 2021, where he joined as manager of talent acquisition and was promoted to director of talent acquisition in 2023. Not-for-profit Emerald Communities, senior housing organization serving Emerald Heights and Heron’s Key, has announced the appointment of Chris Lucero as the executive director of Heron’s Key.

In his role, Lucero will oversee the management, planning, organization, leadership, evaluation and control in all departments of Heron’s Key, according to a press release.

Lucero brings more than 17 years of experience to the role, with the press release stating he has gained strong skills in team leadership, occupancy development, strategic planning, operations management and customer care.

The Residence at Boylston Place names executive director

The Residence at Boylston Place, a LCB Senior Living operated community, named Eileen Mahoney as its executive director on April 2.

In her role, Mahoney is responsible for overseeing the community’s day-to-day operations.

Ventana by Buckner names senior executive director

Brio Living Pivots to Independent Living With Community Expansions, New Lifestyle Brand

Brio Living Services has right-sized skilled nursing and is set to pursue independent living development to spur future growth.

The Grand Rapids, Michigan-based senior living provider operates two continuing care retirement communities, two middle-market senior living communities and one affordable senior housing property. The company also offers home care services and has three majority-owned PACE properties.

Total revenue for the company stood around $175 million in 2023, and looking ahead, Chief of Residential Services Nicole Maag said Brio Living Services has a “financial roadmap” crafted by CEO Steve Fetyko for the company’s future bottom-line success.

“It allows us now to go into being proactive versus being in that reactive mode,” Maag said during an interview with SHN at Senior Living 100 in Laguna Niguel, California. “Our focus is to build-out as fast as we can.”

The company is currently expanding independent living at select properties and has relaunched its Avenues home care program that was shuttered during the Covid-19 pandemic.

Brio Living Services is also piloting programming and branded lifestyle offerings at its communities in order to inform a wider rollout down the road, Maag said. That rollout is slated to include a community paramedic program and a new wellness and lifestyle brand called Balanced Living. Brio’s leaders also are focused on expanding the organization’s PACE footprint and adding affordable housing units at those locations in the future.

The expansion into independent living has been driven by demand for an “in-between” IL model that will include approximately 20 units to create a “hub” within the community for a new independent living product, Maag said. The first addition, known as The Ravines: The Village View, is set to open in 2025 following construction starting this summer.

“It’s neither the 100-unit IL or cottages and we can replicate that model over and over again,” Maag said. “It will allow both the intimacy and the community that residents crave with not the huge operating costs that come associated with cottages.”

The push to expand IL was also driven by the erosion of reimbursements from Medicare Advantage plans and the expenses associated with administering higher acuity care, along with senior living prospects seeking more independence, rather than seeking assisted living or skilled nursing services, Maag said.

“It wasn’t due to a lack of referrals, we were getting the same number of referrals but as Advantage shortened that length of stay, we needed to double or triple those numbers to keep up,” Maag said of the pivot to lower acuity.

Chief Human Resources Office Michelle Baldwin Henderson told Senior Housing News that Brio Living’s staffing retention and recruitment is an “operational, financial and marketing issue” with its applicant flow having more than doubled since 2020.

“We have to make sure that we are responsive and figure out how we can keep people beyond those first 24 months,” Baldwin Henderson said. “We have to make sure that we are onboarding effectively and retaining correctly.”

Looking ahead, Maag said steps taken to transform Brio’s care continuum and efforts on staffing will help create stability and lead to the organization “to start to be proactive and creative.”

“It’s looking towards the future and how to capitalize on the opportunity in front of us and I believe we are poised to do that,” Maag said.

New Washington Post Investigation Centers on Brookdale Senior Living Staffing Algorithm

Staffing algorithms like one used by the nation’s largest senior living operator are exacerbating work shortages and often underestimating the amount of care that older adults need, according to a new investigation from the Washington Post.

The latest story, published Monday, centered mainly on a staffing algorithm used by Brookdale Senior Living (NYSE: BKD) known as “Service Alignment.” According to the Post, the algorithm helped lead to understaffing at the company’s communities, and in the process “failed to capture the nuances of caring for vulnerable seniors.”

“When Service Alignment was rolled out, the algorithm forced many Brookdale facilities to reduce their staff, documents and interviews show. At other facilities, where the algorithm suggested more staff was needed, Brookdale intervened,” wrote Post reporters Douglas MacMillan and Christopher Rowland.

The Post’s reporters based their story on interviews with more than 35 current and former Brookdale employees, court records and internal documents.

In a statement provided by a Brookdale spokesperson, the Brentwood, Tennessee-based company disagreed with the Post’s account.

“Service Alignment is a resource to help guide community leaders on staffing decisions, but local leaders have autonomy and discretion to adjust staffing levels as necessary to meet resident needs. It was created in the early 2000s as a way to identify best practices to meet the needs of residents. Service Alignment does not dictate staffing maximums or determine care costs,” the company’s statement read.

The Post’s investigation on Brookdale’s staffing algorithm is the latest in a series of stories called “Memory Inc.” casting a spotlight on the U.S. assisted living sector. A previous story, published in December, 2023, centered on thousands of cases since 2018 where senior living residents wandered away from their communities, resulting in nearly 100 deaths in that time.

That story sent shockwaves through the industry shortly after it was published, and prompted a U.S. Senate committee hearing in January.

Brookdale is the largest senior living operator in the U.S. with 652 communities nationwide.

Algorithm story adds to industry scrutiny

According to the Washington Post, Brookdale’s staffing algorithm has led to consternation at the community level. Managers “repeatedly complained that the company’s algorithm underestimated the amount of labor they needed to meet resident needs, according to court records,” the Post story noted.

Sometimes, the algorithm led to substandard care for older adults with dementia, the Post found after talking with a handful of the company’s former executive directors.

“Service Alignment failed to capture the complexities of working with seniors with cognitive decline,” the Post reporters wrote. “For instance, helping dementia patients take showers may take two or three times as long as other residents, because they often refuse to get undressed in front of caregivers they may not remember and need to be patiently guided through the process.”

The Post also detailed two civil lawsuits against Brookdale in Tennessee and California in which “a dozen residents or relatives of residents claim they suffered due to short-staffing caused by an overreliance on algorithms.” Sunrise Senior Living also is defending itself in a lawsuit by a group of customers with “similar concerns,” according to the Post.

Brookdale told the Post the claims were “inaccurate.” Brookdale also said that the staffing allegations are “largely derived from purported class action litigation that we and a number of other senior living providers have been facing over the last several years driven by plaintiffs’ attorneys seeking to earn fees.”

“Brookdale disputes the allegations and has been successful in defending against them to date,” the company’s statement read.

Brookdale is not the only company that has undergone legal and media scrutiny in the last 12 months over use of an algorithm. The UnitedHealth Group (NYSE: UNH) is facing a federal lawsuit over UnitedHealthcare’s use of an artificial intelligence tool in making Medicare Advantage claims denials for patients in extended care facilities.

In general, the senior living industry is facing increased scrutiny from multiple sides in 2024, including from the media and local and federal regulators. In response, industry associations, including Argentum, have said that such scrutiny is not warranted given senior living operators’ outsized impact on the health and wellbeing of residents.

“By relying on isolated events and anecdotal reporting, the Post mischaracterizes the experiences of the vast majority of the 1.4 million Americans who reside in assisted living communities today,” Argentum CEO James Balda said in a statement Monday, referencing the April 1 Post story.

“The Post fails to acknowledge that older adults who live in senior housing communities live longer, receive more healthcare services, and benefit from greater rehabilitative and preventive care than those who do not,” Balda continued. “In addition to fulfilling and often exceeding minimum state staffing requirements, Argentum and its members are committed to maintaining sufficient qualified staffing based on the needs of residents, which is central to the concept of person-centered care.”

Integral Senior Living CEO: Partnership With Discovery Has Been ‘Game-Changer’ for Operations

Since partnering with Discovery Senior Living, the past nine months have been a whirlwind of both integration and growth for Integral Senior Living — just ask CEO and President Collette Gray.

“A lot has happened in the last nine months,” Gray said recently during an interview with Senior Housing News on the sidelines of the Senior Living 100 Conference in Laguna Niguel, California. “We have been able to take the best of ISL and the best of Discovery and bring them together to create something truly unique.”

By joining the Discovery Senior Living umbrella, Integral – also known as ISL – has gained access to support functions that have helped bring its business capabilities to a new level and have served as a “game-changer,” Gray said.

While creating change is difficult, Gray said the arrangement with Discovery and the Bonita Springs, Florida-based operator’s corporate support center has shifted the burden away from communities and led to better care outcomes and staffing efficiency.

ISL also has access to an on-call legal team via Discovery’s support center that can assist when needed, something the company didn’t have before, Gray added.

“Together we are one family,” Gray said. “We have more robust tools to help us make decisions that ultimately drive better results and outcomes for our owners.”

The partnership between ISL and Discovery was announced until early February 2023 following an investment by Lee Equity Partners and Coastwood Senior Housing Partners into Discovery. That transaction was completed in June of last year. The result created the nation’s fifth largest senior housing operator, and also followed a recapitalization of Discovery Senior Living by Coastwood in October of 2022.

The recapitalization added fuel to the fire for Discovery’s horizontal growth strategy, and in August of last year, resulted in the launch of LakeHouse Senior Living, a new regional management company aimed at growing the brand’s presence in the Midwest.

Scale, support from Discovery a ‘game-changer’ for ISL

Now Discovery’s largest, vertically integrated senior living operator, ISL exists within the Discovery family of companies, with ISL having management of over 113 communities.

ISL is continuing to grow its brand across its footprints in 24 states. As president and CEO of ISL, Gray continues to lead the Carlsbad, California-based organization while also sitting on the Discovery Senior Living board of trustees.

As a result of the integration, some ISL teams have been elevated to the corporate level to support communities outside of the ISL portfolio, with an in-house recruitment team now providing insights to communities for Discovery and beyond, Gray said.

“We’re taking the good things that are happening from both companies and building them out and growing them so our support services platform is robust and I would say it’s one of the biggest things to have happened as a result of this transaction,” Gray told SHN.

The new support has been a “game changer” for ISL’s operations, specifically for HR-related tasks, Gray said. The company has improved employee retention and recruitment by enacting an improved 401(k) program for staff with an employer match.

“As we are growing and as we are bringing all of those things into the parent company, our staff can focus on the day-to-day and deliver the resident experience and drive results,” Gray said.

On Monday, Gray confirmed that long-time Brookdale Senior Living (NYSE: BKD) executive Laura Fischer has started in her new role as COO for ISL. According to her profile on the Brookdale website, Fischer was previously the Division Vice President for Brookdale’s West division, joining Brookdale in 2017. She brings over two decades of senior living experience to her new role with ISL.

The ecosystem of Discovery’s deep bench of executive leadership across its regional brand landscape has allowed ISL to find best practices in operations and glean new ideas to improve resident and employee experiences.

“We’re able to talk about best practices and find out what’s working and it’s a different way to grow,” Gray said.

Improving operations to evolve in 2024

While operators remain challenged by headwinds in new development this year, many have found new ways to grow in 2024, from campus redevelopment projects to acquiring and turning around communities at a discount to replacement costs. ISL has remained somewhat active on development based on the assumption that demand in the future will ease the headaches of pushing through construction in today’s higher-interest climate.

For 2024, Gray said ISL would continue to find “operational excellence” as the integration continues with Discovery. The company will also add two new developments set to open later this year in Texas and Nevada.

“It is all about evolution,” Gray said. “We are focused on driving results and improving the resident and associate experience.”

At the time of the announcement, Discovery CEO Richard Hutchinson highlighted the long-term vision to build out a national and regional brand management structure that will allow for what he has called sustainable scaling, as previously reported by SHN.

“This wasn’t something where we were just trying to get a bag of money and run off into the sunset,” Hutchinson said. “It was evolutionary and purposeful and thoughtful in its timing, and we really just needed some juice to take it to that next level.”

During a July 2023 edition of SHN+ Talks, Hutchinson noted how ISL and Discovery were relying on “best-in-class, world-class integration consultants” to help ease the transition between both organizations.

Thanks to the help from the companies’ corporate support center, ISL teams at the community level can monitor real-time data regarding resident health, length of stay, occupancy and more.

“We can project outcomes based on what’s happening now and in real-time, and we slice it and we have a whole team looking at projections on things to make improvements where we can,” Gray said.

Gray said operators must continue to innovate on technology integration as ISL and Discovery have in recent years.

“There’s more of a strategic plan around technology I think more so than there ever has been,” Gray said. “That will help drive results and if our teams can focus on the core business, which is taking care of the residents, it all makes sense.”

With the changes in place that will allow for future growth, Gray said the evolution between ISL and Discovery will help teams work “smarter not harder.”

Gray’s perspective when weighing ISL’s operating margins is informed by how rising acuity of residents and length of stay is creating a “new normal” for many operators.

“You’re going to have higher acuity and you’re going to need additional staffing today,” Gray said. “You’re also going to have to change your price offering and determine service levels that change alongside increasing expenses, and it’s just managing to that fine line.”

Lutheran Senior Services, Diakon to Join Forces in Bid for Greater Scale

Senior living operator Lutheran Senior Services (LSS) has signed an agreement to affiliate with another nonprofit in a move that CEO Adam Marles said will help the organization better prepare for the coming two decades.

LSS recently announced its intent to affiliate with the senior living communities operated by Diakon, a Lutheran-aligned senior living operator based in Pennsylvania. With the move, LSS is adding four life plan communities to its portfolio, which will encompass a dozen life plan communities, one assisted living community and nine affordable senior housing communities when all is said and done.

Marles, who has led St. Louis-based LSS since 2021, said the decision was driven partly by a belief that “scale is going to matter for the success of non-profit senior living moving forward.”

“It will make sure that we are better able to compete for talent; make sure that we can continue to add and to grow specialty positions within the organization; create career-ladder opportunities for our team; and then obviously, increase purchasing power, negotiating power, spread overhead, improve access to capital,” Marles told Senior Housing News.

The newly combined organization would have a total of more than 3,500 team members with more than 5,300 residents and rank as the 12th largest non-profit senior living operator, at least according to the most recent LeadingAge Ziegler 200 report, released last year.

Factors going into the agreement, according to Marles, includes Diakon’s long term “legacy of serving their residents well” and the financial stability of both organizations

“This is a transformative moment in the 165 year history of Lutheran Senior Services,” Marles said. “We collectively are really excited about what this will mean for our organization moving forward and set us on a trajectory to continue our growth, continue to improve all of our services, while maintaining our our quality and and our ability to care not only for the people we serve, but also the employees of LSS.”

Diakon-managed communities are keeping their respective names in the merger. The organizations expect to finalize the affiliation later this summer.

The announcement follows LSS’ ongoing “pivot year” in 2024, In January, LSS acquired St. Louis Home Health, which Marles said added an orthopedic specialty to the organization’s home health line and brought LSS home health services into Illinois. In February, LSS acquired New Horizons PACE, a Program of All-Inclusive Care for the Elderly (PACE) program in the St. Louis region.

“Home health, PACE and life plan communities all serve people in different types of ways, and we believe that diversification of services is going to be critical to our long term success,” Marles said.

Additionally, in 2022 LSS created a new strategic framework for the organization with four primary goals; culture that “brings out the best in each other,” products and services that evolve with changing consumer preferences, growth through partnership and collaboration and a sustainable financial future. This agreement, Marles said, is at the core of the goals laid out by the organization.

“This deal is really a way to continue on with those strategic goals,” Marles said. “It helps us to continue to address the changing needs of our residents and our clients.”
